Key Responsibilities :

- Care Planning : Collaborate with families and healthcare professionals to develop individualized care plans, tailored to meet each client's unique health requirements and personal preferences.
- Patient and Family Education : Offer guidance and support to families, educating them on elder care techniques, proper use of medical equipment, and ongoing care needs.
- Safety and Compliance : Adhere to safety protocols and infection control procedures, ensuring a safe and clean environment for clients. Maintain up-to-date documentation of care provided in compliance with homecare standards.
Qualifications :
Certification and Education :
- Nursing graduate (Bachelor’s degree or diploma) with experience in elderly care; OR
- Registered Nurse (RN) with experience in elderly care.
- Experience : A minimum of 2 years in a healthcare setting focused on elderly or homecare services.
Skills and Competencies :
- Strong understanding of elderly care needs, including chronic condition management and personal assistance.
- Excellent communication and interpersonal skills, with a compassionate, patient-centered approach.
- Ability to work independently and provide high-quality care in a home setting.
