• Maintains active communication with physicians, nursing staff, and other team members for effective patient care management.

• Determines Correct Level of care placement- utilization management.
• Provides accurate medical documentation and health insurance documentation.

• Acts as a liaison between clinicians and revenue departments.
• Identifies and resolves variances to clinical pathways and obstacles to discharge.
• Consults from appropriate disciplines/departments as required expediting care and facilitating discharge.
• Ensures patient care is rendered to the maximum stipulation based on the patient's benefits under the Insurance contract without exceeding the providers' provision for the cost of care.
• Monitors and intervening as necessary to achieve desired goals and outcomes for both the patient and the hospital.
• Assesses and intervenes to address psychosocial needs including patient, family and community and collaborates with social workers as appropriate.
• Supports physicians in relation to Clinical Documentation impacting Medical Necessity, Case Mix Index, and Severity of Illness.
• Monitors length of stay and resource use on an ongoing basis and discusses trends with Senior Case Managers.
• Assists in the collection and reporting of indicators including LOS, excess days, resource utilization, and readmission rates.

• Monitors the patient's progress, intervening as necessary and appropriate to ensure that the plan of care and services provided are patient-focused, quality efficient, and cost-effective.
• Plans utilization of resources and ensures the available funding meets the clinical needs.
• Facilitates along the Continuum of Care, monitoring against Clinical Pathway, and facilitation of transition plans.
• Ensures proactive discharge planning.

• Required: Bachelor's Degree in Nursing, Registration as a Registered Nurse from Canada, USA, UK, Ireland, South Africa, New Zealand, and Australia, Valid/Current national license, BLS.
• Desired: Experience in Large Healthcare facility.

Role and Responsibilities:

• Support care delivery to patients - Communicate and provide resources to patient and caregivers about patient's conditions and treatment requirements - Recognize and appropriately manage challenging behaviors of patient and caregivers - Liaise and follow through with patients and all health and social care providers to keep everyone informed and updated
• Organize patient information for efficient data transfer to relevant internal staff and external providers - Receive and collate information from new, existing and transfer patients - Liaise with other providers to ensure patients at risk are monitored adequately - Conduct checks on data input to ensure quality and consistency
• Generate reports for population health management
• Coordinate among multidisciplinary teams in the provider's setting - Liaise with clinical and non-clinical staff for effective patient management - Organize and manage minutes of internal meetings as needed - Follow up and tracking of preventive screening results to ensure the timeliness and completeness of follow-up as specifications and care-pathway Key Interactions:
• Internal Interactions - Clinical lead - Physicians within the facility - Administrative within the facility - Other clinically trained personnel such as paramedics, technicians, nurses, etc
• Patient and Visitor Management: Greet and check in patients, who are often employees or contractors. They verify identity, manage a multi-line phone system, and direct visitors. This is the primary point of contact for anyone entering the clinic
• Appointment and Schedule Management: Book, confirm, and reschedule appointments for medical consultations, pre-employment screenings, drug and alcohol testing, and injury follow-ups. They coordinate with medical staff to optimize the clinic's workflow
• Medical Records and Data Entry: Maintain and update confidential patient records, including employee medical histories, injury reports, and treatment plans
• Administrative and Clerical Support: Handle a variety of office tasks, such as filing, scanning, and distributing mail. They also manage clinic inventory, ordering medical and office supplies as needed to keep the clinic operational
  

• Diploma or Degree in Health Care Administration, Public Health, Health Education
• Minimum 2 years' experience as a patient care coordinator
• Strong customer service skills with a friendly and approachable demeanor
• Excellent communication skills in English
